MCCCHD Immunizations ProgramChild Immunization Program Children need protection from serious diseases. Immunization (vaccination) is a way of creating immunity to certain diseases. The risks of vaccinations are extremely small compared with the health risks and diseases that can occur if not immunized.
- Bring your child’s current vaccination record.
- Medicaid accepted.
- No one is denied VFC (vaccine For Children) due to inability to pay.
- Child must have a parent accompany them, or bring signed screening/consent form (signed by parent or guardian). This form can be downloaded below. This form can also be sent to the health dept. by mail or fax also. MCCCHD Screening/Consent Form
